Hook: hard fail on toolchain install errors
Replaced warn-and-continue pattern with set -euo pipefail and die(). If a toolchain install fails, the session cannot build — stop immediately. Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
